Guidelines based on a study of the sociodemographic characteristics of the four-county Metropolitan Health Planning Corporation (Cleveland, Ohio) area and of past - projected utilization of area hospitals' obstetric and pediatric services are presented for use by area facilities in planning and providing inpatient obstetric and pediatric services. It is noted that existing inpatient facilities for obstetric and pediatric care are sufficient to service expected demand. Consolidation among existing obstetric units is encouraged to maintain a minimum size unit of 33 beds and to assure that geographic distribution meets projected patterns of demand. Smaller units should have an average annual occupancy rate of 80 percent; larger units may have a higher rate. Pediatric units should maintain a minimum of 20 beds; consolidation and merger among the existing units at 28 area hospitals is encouraged to facilitate this. Smaller pediatric units should operate at 75 percent occupancy; larger units should have a higher occupancy rate. A limited number of regionalized pediatric referral centers offering tertiary care are to be designated within the region. Each referral center must serve a minimum number of pediatric patients equivalent to 8,500 patient days and must demonstrate fiscal and facility resources to support its staff and equipment.
